What Is Skeletal Muscle Mass (SMM)?

Skeletal muscle mass (SMM) is the total weight of the muscles attached to your skeleton — the muscles you use to move, lift, and maintain posture. Unlike smooth muscle (in organs) or cardiac muscle (the heart), skeletal muscle is voluntary and responds to training and nutrition.

SMM is important because low muscle mass is linked to sarcopenia (age-related muscle loss), frailty, falls, insulin resistance, and higher mortality. Tracking SMM — or its height-normalized index, SMI — helps identify people at risk before function declines.

How This Calculator Estimates SMM

Direct measurement of SMM requires MRI, CT, or DXA. For everyday use, validated anthropometric equations estimate appendicular skeletal muscle mass (ASM) from basic inputs: age, weight, height, and sex.

Anthropometric (Lee-style) equation — default:

ASM (kg) = 0.193 × weight(kg) + 0.107 × height(cm) − 4.157 × sex − 0.037 × age − 2.631

Sex: male = 1, female = 2. SMM ≈ ASM × 1.15 (to approximate total-body skeletal muscle).

Height × weight (Janssen-style) equation:

SMM (kg) = −0.003 × weight² + 0.772 × weight − 10.100

This simplified weight-based estimate is useful as a cross-check.

Skeletal Muscle Mass Index (SMI):

SMI (kg/m²) = SMM (kg) ÷ height² (m²)

SMM % of body weight = (SMM ÷ weight) × 100

The calculator also shows BMI as a secondary reference so you can interpret your SMM in the context of overall body size.

Understanding SMI and Sarcopenia Cutoffs

SMI normalizes muscle mass to height, allowing comparison across people of different sizes. The most widely used sarcopenia cutoffs (Janssen et al., 2002, 2004) are:

  • Men: SMI < 7.26 kg/m² indicates low muscle mass (sarcopenia).
  • Women: SMI < 5.45 kg/m² indicates low muscle mass (sarcopenia).

Some studies also use SMI as a percentage of body mass: low muscle mass is defined as ≤ 37% in men and ≤ 28% in women (Janssen class 1 sarcopenia). This calculator uses the kg/m² SMI as the primary sarcopenia screen and shows your percentage for reference.

Why Use This Skeletal Muscle Mass Calculator?

  • Validated equations: Uses anthropometric formulas derived from MRI-validated reference methods.
  • Two estimation methods: Compare a Lee-style ASM equation and a Janssen-style weight equation.
  • Sarcopenia screening: See your SMI against clinical cutoffs for men and women.
  • Visual breakdown: A chart shows your muscle mass relative to total body weight.
  • Detailed breakdown: Every input, formula, and intermediate value is shown.
  • Free & Private: No registration, no data storage.

Understanding Your Muscle Mass Category

  • Low: SMM or SMI is below the sarcopenia cutoff — further assessment (strength, function) is recommended.
  • Moderate: SMM is in the lower half of the normal range — maintain or build muscle with resistance training and adequate protein.
  • Optimal: SMM and SMI are within healthy ranges for your sex and age.
  • High: SMM is above average — typical of trained individuals or athletes.

Note: Anthropometric equations estimate SMM indirectly. For clinical diagnosis of sarcopenia, combine SMI with grip strength and physical performance tests (e.g., gait speed, chair-stand) as recommended by EWGSOP and AWGS guidelines.

❓ Skeletal Muscle Mass Calculator FAQ

What is skeletal muscle mass (SMM)?

SMM is the weight of the muscles attached to your skeleton — the muscles you use for movement, posture, and lifting. It's distinct from smooth muscle (organs) and cardiac muscle (heart).

How is SMM estimated without MRI or DXA?

Validated anthropometric equations estimate appendicular skeletal muscle mass (ASM) from age, weight, height, and sex. These equations were derived by comparing simple measurements to MRI or DXA in large study populations.

What is the skeletal muscle mass index (SMI)?

SMI is your skeletal muscle mass divided by your height squared (kg/m²). It normalizes muscle mass for body size, making it comparable across people of different heights.

What SMI value indicates sarcopenia?

Common cutoffs are SMI < 7.26 kg/m² for men and < 5.45 kg/m² for women. These thresholds were associated with increased physical disability risk in older adults.

How accurate are anthropometric SMM equations?

They explain roughly 80–90% of the variance in MRI-measured muscle mass in validation studies. Accuracy is lower in very obese, very lean, or highly trained individuals, and in populations different from the derivation cohort.

Does this calculator work for athletes?

It can give a rough estimate, but anthropometric equations tend to underestimate muscle mass in highly muscular athletes. For precise tracking, DXA or BIA with athlete-specific settings is preferable.

Can I use this calculator if I'm under 18?

The equations are validated for adults (18+). Children and adolescents have different body proportions and growth patterns — use pediatric-specific references or consult a clinician.

What is the difference between SMM and ASM?

ASM (appendicular skeletal muscle mass) is the muscle mass in your arms and legs. SMM is the total skeletal muscle mass of the whole body. The calculator estimates ASM and multiplies by ~1.15 to approximate total SMM.

How can I increase my skeletal muscle mass?

Resistance training (2–4 sessions per week), adequate protein intake (1.6 g/kg/day), sufficient calories, and good sleep are the main drivers of muscle growth. Progressive overload is key.

Does muscle mass decrease with age?

Yes — after about age 30, adults lose roughly 3–8% of muscle mass per decade, accelerating after 60. This is why resistance training and protein intake become increasingly important with age.

Is SMI the same as BMI?

No. BMI is weight ÷ height², which does not distinguish muscle from fat. SMI is muscle mass ÷ height², which specifically reflects muscle. A person can have a normal BMI but low SMI (sarcopenic obesity).

How do I convert measurements between metric and imperial?

This calculator automatically converts between metric and imperial units. Simply select your preferred unit system, enter your measurements, and the calculator handles the conversions for you.
